Pension Forms and Retirement Applications

 Retirement Application Packets:

IMPORTANT! If you are intending to retire, you must request an official retirement benefit estimate approximately six months prior to your expected retirement date. Once you have received your estimate, please download the necessary application packet for the appropriate retirement plan. You may complete the application forms on-line, print the completed forms, sign and have notarized where needed, and send the completed forms to our office for processing. Your benefit will commence when your employer confirms your termination and final contributions are received. Failure to obtain an official estimate in advance, could significantly delay your benefit.

Public Employee Forms

Please note, these forms are for the Public Employee Pension. You will see next to the document whether you can use it for both Pension and Deferred Compensation plans or if you have to use a separate document for each plan.

Change Forms for Active Members

• (WRS-3) Account Information - Request changes to the name or address on your Pension and/or Deferred Compensation account.
• (WRS-2) Beneficiary Information - Request changes to the beneficiary on your Pension account only.

Account Refund Forms

Non-Vested Members (less than 48 months of service)

• Refund Instructions and Checklist- Please select this option only if you have less than 48 months of service in the Wyoming Retirement System and want a refund or rollover of funds. For Pension only.

Vested Members (48 months of service or more)

• Account Refund: Vested Member - There are certain things you need to know about getting an account refund if you have a total of 48 or more months of contributions in the Wyoming Retirement System (WRS). Most importantly, you will forfeit your right to a monthly benefit at retirement. To fully understand the lifetime benefit you are forfeiting, you must obtain an official benefit estimate from WRS before you may take a refund on your account. • (WRS-B1) Retirement Estimate Request - Use our On-line Benefits Calculator for a faster estimate of your benefits. Pension only.

 

Volunteer Firefighter Forms

• (WRS-2VF) Volunteer Fire Change Notification - Volunteer Firefighter Account change request including Name, Address, and Beneficiary.
• Volunteer Fire Refund Instructions and Checklist -Use this packet when you want to withdraw your contribution or to roll your WRS Pension into a different account.

EMT Forms

• (WRS-2EMT) EMT Notification - EMT Account change request including Name, Address, and Beneficiary.  

Retiree Forms

• (WRS-3) Account Information - Request personal changes on your account including name and address. Please contact WRS for changes to beneficiary information.
• (WRS-A8) Automatic Payroll Deposit - Initiate the electronic deposit of retirement benefits or change financial institutions.
• (WRS-A9) Income Tax Withholding Request - Request change in amount of Federal Income Tax withheld from your benefit.
