Employer Forms for Pension Plan

(WRS-1) Registration Form for All Agencies
(WRS-6) State Agency Transfer Form - This form replaces the Notice of Termination and Registration forms for employees tranferring to a different agency within the State of Wyoming.
(WRS-7) Termination Form - Notice of Termination of Employment.
(WRS-11) Payroll Adjustment Form - All adjustments must be pre-approved by the WRS Accounting Department PRIOR to sending to WRS.
(WRS-13) Notice of Unpaid Leave of Absence - If an employee is on worker’s compensation, FMLA, or any other type of extended leave, please submit this form.
(WRS-13A) Notice of Unpaid Leave of Absence Salary Information Request - This form should be completed if the employee is returning from Military Leave or Workers' Compensation Leave. Please include the WRS-13 with this form.

(WRS-1VF) Volunteer Fire Registration - Registration Form to participate in the Volunteer Firemen's Pension Fund
(WRS-32VF) Volunteer Fire Notification of Participate - Agreement form to Participate in the Volunteer Firemen's Pension Fund
(WRS-7VF) Volunteer Fire Notification of Termination/Notice of Suspension - Notice to Terminate from the Volunteer Firemen's Pension Fund
(WRS-VFRemit) Volunteer Fireman Remittance Form - Initial enrollment or additional payment for new members (For Employers)

(WRS-1EMT) EMT Registration - Registration Form to participate in the EMT's Pension Fund
(WRS-32EMT) EMT Notification of Participate - Agreement form to Participate in the EMT's Pension Fund
(WRS-7EMT) EMT Notification of Termination - Notice to Terminate from the EMT's Pension Fund
